How to Register
Step 1
Submit the form
Fill out the student and parent information below. An approximate NYSSMA level is fine.
Step 2
Pay the fee
Use Zeffy or mail a check. Include the student's full name so the payment can be matched to the registration.
Registration Form
Having trouble with the form? Email your registration information to info@ayoa.org. You can also open it directly in a new tab here.
If the form asks for your NYSSMA level, an approximate level is fine.
Registration Fee
After submitting the registration form, pay online through Zeffy or send a check by mail.
On the Zeffy payment form, choose the correct session and enter the student's first and last name so AYOA can match the payment to the registration.

- Instrument and NYSSMA level (approximate)
- School and graduating year
- Private teacher and teacher phone
- Ensemble interest and fee details
- Student name included with payment
- Parent consent and date
